Biannual update shows more residents connected to care and permanent housing

Sacramento County’s latest biannual report shows growing success in its efforts to connect unhoused residents with housing, shelter, and behavioral health services. The report, presented this week to the Board of Supervisors by the Departments of Homeless Services and Housing (DHSH) and Health Services, covers outreach from January through June 2025 across the unincorporated county and the American River Parkway.

This is the fourth update since the City and County of Sacramento formed a joint homelessness partnership in late 2023. County outreach teams — including parkway, encampment, and community-based staff — provided 31,355 services to 771 individuals and helped 203 transition out of unsheltered homelessness. Officials noted that while fewer people were served overall, the number of successful placements increased.

Services included general case management, housing placement, document and healthcare support, and food assistance. More than half of those served identified as BIPOC, and the majority were between 25 and 61 years old.

Behavioral health referrals also rose, with 1,569 unhoused residents referred for care and 1,080 successfully linked to services — a significant jump from the previous six months. The County’s HEART Team engaged more than 1,600 individuals, connecting 178 to outpatient programs.

In all, Sacramento County invested $277.4 million in homelessness initiatives during fiscal year 2024–25, including $39 million for new Safe Stay shelters and $154 million in funding for community-based organizations. Nearly half of all funding came from state sources, with additional support from ARPA, federal, and local funds.

DHSH continues to expand its public dashboards tracking outreach, shelter, and re-housing outcomes, offering transparent insight into demographics and program performance. The data, county officials say, supports ongoing improvements in how Sacramento addresses homelessness.
