The Sacramento County Departments of Homeless Services and Housing, and Health Services, presented their biannual report to the Board of Supervisors this week, detailing outreach and engagement efforts to the unhoused population in unincorporated Sacramento County and along the American River Parkway from July 1 to Dec. 31, 2024.

This marks the third update since the launch of the City/County Partnership agreement signed in December 2023. While the collaboration involves both City and County efforts, the latest report focuses on work conducted by County teams outside of Sacramento city limits.

Thousands Reached Through Outreach Services

Between July and December, county outreach teams—comprised of 10 parkway staff, 10 encampment services staff, and six community-based outreach workers—provided 25,274 services to 810 unduplicated individuals experiencing homelessness. Among those served, 184 individuals transitioned out of unsheltered homelessness into shelter or housing.

The data averages out to 31 services per individual during the six-month period. Examples of services provided included 16,397 instances of general case management, 1,206 housing search or placement services, 1,024 instances of vital document support, 393 connections to healthcare services, 294 CalAIM support services, and 351 Coordinated Access System (CAS) referrals.

Demographic data revealed that over 70% of those served were between the ages of 25 and 61. Of those, 53% identified as male and 53% as BIPOC (Black, Indigenous, or People of Color).

Shelter Demographics Highlight Long-Term Homelessness

In County-funded shelters, 1,137 unique households were enrolled during the reporting period. The data shows that 45% of shelter residents were between ages 45 and 64, 64% reported a disabling condition, and 40% had no income. On average, these individuals had experienced homelessness for nearly four years.

Behavioral Health and HEART Team Efforts

From July to December 2024, 1,360 unhoused residents were referred to behavioral health services, with 947 successfully linked to care. During the same period, the County’s Homeless Engagement and Response Team (HEART) engaged 1,607 individuals, made 342 referrals to behavioral health programs, and successfully connected 171 people to outpatient services.

Full Service Partnerships Continue to Support Housing Stability

Sacramento County’s Full Service Partnerships (FSPs), which offer intensive case management and mental health support, served 2,631 individuals during the reporting period—95% of the program’s total capacity. Among those served, 76% were housed, 11% were in shelters, 9% remained unsheltered, and 4% had unknown housing status.

Data Dashboards Offer Transparency and Insight

The Department of Homeless Services and Housing continues to update public-facing data dashboards that reflect key metrics, including program utilization, service types, and demographic details. The dashboards are organized into two main categories: Program Performance metrics and Demographic Insights. Together, they offer a comprehensive view of the County’s ongoing efforts and allow for data-driven improvements in service delivery.
