Mid-century-inspired dementia care concept targets April 2026 opening in long-vacant Oak Avenue Parkway site
Construction is officially underway on a new adult day care center in Folsom that promises to bring a one-of-a-kind approach to senior care, particularly for individuals living with dementia, while also breathing new life into a long-vacant commercial space.
Town Square Innovative Adult Day has begun construction on its newest location at 9580 Oak Avenue Parkway, Suite 4B, the former Vic’s Supermarket site near American River Canyon that has stood empty for years and has been a point of anticipation for the surrounding community.
The project marks a major milestone for local owners Nick and Desiree Trunzo, who are introducing Town Square’s nationally recognized reminiscence-based adult day care model to Folsom.
The center is designed as a fully immersive, mid-century American town, complete with themed storefronts that include a diner, movie theater, city hall, art studio and health club. Each space is intentionally created to spark memory recall, encourage social interaction and provide meaningful, structured activities for seniors, with a strong focus on those experiencing Alzheimer’s disease or other forms of dementia.
“Breaking ground on our new center is an exciting step toward serving families in Folsom,” said Nick Trunzo, co-owner of Town Square Folsom. “We look forward to creating a space where older adults can thrive and families can find peace of mind knowing their loved ones are in a safe, engaging environment.”

The concept behind Town Square is rooted in reminiscence therapy, a proven care approach that uses familiar sights, sounds and routines from earlier decades to reduce anxiety, support cognitive health and foster emotional connection. By recreating the look and feel of a mid-20th-century town, the centers aim to provide seniors with a comforting sense of purpose and belonging throughout the day.
For Desiree Trunzo, the location itself carries personal significance, adding another layer of meaning to the project.
“It feels especially fitting that we’re opening this nostalgic environment in a space that holds such meaningful memories for me,” she said. “Growing up in Orangevale, I remember my grandparents taking me to Vic’s Supermarket for the best sandwiches. This building will once again be a place where families create memories.”
Beyond its impact on families seeking daytime care and respite, the project is also expected to provide an economic boost to the area. Local contractors and vendors are contributing to the buildout as the long-dormant property is transformed into an active, community-serving business.

The Folsom location is notable as there are currently no other adult day health care centers in the immediate area dedicated specifically to serving seniors with dementia, making Town Square a unique addition to the city’s senior care landscape.
